Requirements:
Passport of seaman/seafarer : 2 copies
Must be at least 6 months past contract expiration date
Valid Seafarer’s Identification and Record Book (SIRB)
Proof of previous employment
Original copy of employment contract
Original copy of Basic Seaman Training certificate
Seafarer registration certificate
NBI clearance original and copy
OFW info-sheet/ E-card information form
Pre-departure orientation seminar ( PDOS)
Duly accredited DOH medical examination and copy
Certificate of proficiency from NAC or COC from Tesda
Plane ticket or booking certificate
For Visa Processing:
Duly accomplished visa application form with 2 pcs. 2×2 picture
Copy of visa authorization / availability
Sea-Based Contract Processing Division
Mezzanine Floor Procedure at POEA
Step 1: Submit complete documents to the evaluator at counter 1.
Step 2: Go to the releasing counter for the release of encoded documents.
Step 3: Pay corresponding fees to the Cashier and get e-receipt/exit clearance.
Processing Time:
Four hours per request for processing fees
